National digital identity-enabled health record platform

African countries still use paper-based medical records. Patients often visit different hospitals without a shared medical history. This issue leads to repeated tests, delayed treatment, duplicate prescriptions, and weak follow-up care, especially in rural areas. The proposed platform links health records to the national digital ID system and creates a digital health profile for each citizen. Healthcare providers verify patient identity through biometrics or OTP-based authentication and access medical records instantly. The platform stores consultations, prescriptions, vaccination history, laboratory reports, and treatment details in one trusted system. Citizens access records through a mobile application and share data securely through quick response (QR) codes or consent-based access. Healthcare workers can update records online or offline, with automatic synchronization when an internet connection is available. Encrypted data exchange and audit trails protect patient information.
